Warfarin Reinfarction Study
Authors: Smith P, Arnesen H, Holme I
Titles: The effect of warfarin on mortality and reinfarction after myocardial infarction
References: N Engl J Med 1990;323:147-52
Disease: MI
Purpose: To determine the effects of warfarin on mortality, reinfarction and cerebrovascular events after MI
Study Design: Randomised, double-blind, placebo- controlled
Follow-up: Average 37 months
Patients: 1214 patients (607 warfarin and 607 placebo), who had recovered from MI, aged < 75 years. Mean time since MI: 27 days
Treatmentregimen: Warfarin adjusted to prolong prothrombin time to achieve an international normalised ratio between 2.8 and 4.8
Results: The total number of deaths was 94 patients in the warfarin group and 123 patients in the placebo group, representing a 24% reduction with warfarin (p = 0.02). Reinfarction occurred in 82 patients in the warfarin group and 124 patients in the placebo group, representing a 34% reduction with warfarin (p = 0.0001). Cerebrovascular events occurred in 20 patients in the warfarin group and 44 patients in the placebo group, representing a 55% reduction with warfarin (p = 0.0015)
